OHEL is seeking a talented, self-motivated and enthusiastic person to join our Access Team. OHEL's Access department is a single point of entry for those seeking the many services and resources available through OHEL- including people with serious mental illness, children and adults with developmental disabilities, families in crisis, or everyday people with everyday problems. The Access department provides accessibility, relevant information and immediate help to those seeking OHEL’s breadth of services. Primary responsibilities will include processing applications to OMH housing program, developing relationships with referral sources, obtaining necessary paperwork and being a liaison to community hospitals – to ensure seamless transitions around patient discharge, admission into OHEL housing and community integration. Additional tasks include processing referrals in our centralized data base, follow-up with appropriate services, and participating in both in-reach and outreach services. Applicant should possess excellent customer service skills, interpersonal skills, clinical insight and proficient computer skills. BA preferred. This full time position is located in Brooklyn.
